OEM Website Audit Questionnaire
The OEM Audit Questionnaire has been developed by the OEM Audit Steering Committee in cooperation with EPA and has been approved by the Office of Management and Budget (OMB). This questionnaire covers a broad range of topics including website accessibility and navigation as well as the availability of certain service and repair information and is currently being used by the group of technicians that have applied to audit one or more of the OEM websites.
If you have applied to perform an OEM website audit and have received confirmation that you have been assigned to audit a specific website, you may elect to use either the Adobe Acrobat version or the MS Word version of this questionnaire. Procedures for the use and submittal of these files are as follows:
Adobe Acrobat Format - The OEM Website Audit questionnaire was created in Adobe Professional to allow for data entry within the file itself. If possible, the questionnaire should be opened and saved using the latest version of Adobe Acrobat "Reader" (7.0). If you do not already have this version, it can be downloaded at http://www.adobe.com/support/downloads/main.html. Please note that you will only be able to save your responses within the questionnaire as a separate Adobe file if you are using Adobe 6.0 or 7.0 "Standard" (not with any prior versions of Adobe). If you are using any version of Adobe Acrobat "Reader" (i.e., the free download from their website) including version 7.0, you will not be able to save your responses in the questionnaire as a separate file at any point in time during the process of filling out the questionnaire.
For example, using Adobe 7.0 "Reader," it is not possible to fill out a portion of the questionnaire, save the file, and then go back to complete your responses at a later time. Upon completion, you may print your own copy of the questionnaire. To submit your questionnaire, you may use the "Submit by Email" button, which automatically forwards a copy of your completed questionnaire directly to oemaudit@pqa.com. If you are using a "Standard" Adobe version and have saved a copy of your completed questionnaire, as an alternative to using the "Submit by Email" button, you may send a separate email to oemaudit@pqa.com with the questionnaire as a file attachment.
MS Word Format - As an alternative to the Adobe Acrobat file, we are also providing an MS Word version of the questionnaire that you may use. This option may be particularly useful if you do not have a "Standard" version of Adobe, do not plan on filling out the questionnaire all at one time, and/or would like to periodically save your results as you complete each question or segment. You may submit your completed questionnaire via email to oemaudit@pqa.com.
